Can only the G Suite Admin create and manage groups?

No. Anyone with the application can create and manage groups in Share Email Contacts and manage contacts and domain users. Whoever creates a group becomes the Group Admin and that person can manage domain users for that particular group. Click here to know more about #Share Email Contacts for Gmail.
